Musculoskeletal disease of the upper limb that require surgery under regional anesthesia

Drug

To compare the onset of sensory-motor blockade in the ipsilateral upper extremity between two local anaesthetic drugs i.e. “lidocaine” and “ropivacaine” via the costoclavicular space for brachial plexus block (costoclavicular brachial plexus). Lidocaine and ropivacaine are commonly used local anesthetic agents in daily clinical practice for patients requiring to have surgery under local or regional anesthesia

Under ultrasound guidance, patient will receive either 1.5% lidocaine with adrenaline 25 ml, or 0.5% ropivacaine 25 ml injected at infraclavicular brachial plexus

Patient will receive either 1.5% lidocaine with adrenaline 25 ml, or 0.5% ropivacaine 25 ml

once before operation

One group of patients will receive lidocaine with adrenaline and another group will receive ropivacaine.

Patients who are between 20 to 70 years of age, of American Society of Anesthesiologists (ASA) status I to III, and planned for arm, forearm and hand surgery under regional anesthesia in Prince of Wales Hospital.

Patients who refuse regional anesthesia or unwilling to participate in the study; ASA physical status more than III; Pregnant patients; Patients with neuromuscular disorder; Prior surgery in the medial infraclavicular area on the same side of operation; Patients who are coagulopathic or taking anticoagulants; Patients suffering from dysaesthesia or chronic neuropathic pain at the site of surgery; History of local anesthetics allergy; Skin infection over the site of the block.

Proportion of patients achieving complete sensory and motor blockade at 30 minutes

complete sensory & motor block means sensory score =0 and motor score =0

at regular intervals for 45 minutes (5 min, 10 min, 15 min, 20 min, 30 min, 45 min) after the block

Time of readiness for surgery; Time to complete sensory and motor block; Sensory & motor scores at all the time points over all nerve distributions (median, radial, ulnar and musculocutaneous nerve); Proportion of patients achieving readiness for surgery at all time points; Paresthesis & discomfort

readiness for surgery (sensory score≤ 30 and motor score ≤1); complete sensory and motor block (sensory score = 0 and motor score = 0); parethesia (yes or no); degree of discomfort (verbal rating scale, 0-100, 0=no pain & 100= worst imaginable pain)

at regular intervals for 45 minutes (5 min, 10 min, 15 min, 20 min, 30 min, 45 min) after the block
