The e-health mobile apps is an applications that can be installed on smart phone. The mobile apps provide a safe, individualized, interactive platform where members can input, view and retrieve their own blood sugar, exercise records, health measures daily.

6 months

Usual care: general briefing of DM care by nurse, follow-up by physicians.

Ethnic Chinese adult who attends regular follow-up treatments for their DM problems, can read and communicate in Chinese, can use and access Internet at home and willing to participate.

Patients with physical, mental, visual or cognitive impairments, as recorded in their health records, and those who have contra-indications to walking will be excluded from the study.

HbA1c level (at baseline, 3 months, 6 months)

Diabetes self-care activities and self-management; Chronic disease self-management program questionnaire; Diabetes management self-efficacy scale; exercise efficacy; exercise adherence; DM physical parameters and complications (e.g. BP, BMI, lipid profile, DM complications); psychological health outcomes [all above mentioned will be evaluate at baseline, 3 months and 6 months)
